First Visit

As you make your initial appointment, it will be discussed whether this is a consultation appointment or an appointment for surgery. In either case, your diagnosis and treatment options will be explained at the appointment. A complex medical history or complicated treatment plan will require that your treatment be provided at a later date.

Please assist us by providing the following information at the time of your initial appointment:

• Your surgical referral slip and any radiographs
  (x-rays), if not forwarded directly to our office by your dentist.
• A list of medications you are presently taking including prescription medications, doctor's office samples, over-the-counter medications, vitamins, minerals, herbals, and nutritional supplements.
• If you have medical or dental insurance, bring the necessary cards and/or forms. This will allow us to help you process any claims.

Financial Information

Prior to proceeding with treatment, our staff will discuss all financial details with you. Fees will be clearly stated and provided to you in writing. Arrangements for payment will be made at that time.

For your convenience, we accept personal checks, American Express, Visa, MasterCard, and Discover cards in addition to cash. If a payment plan is needed, we participate in the CareCredit healthcare financing program.

For patients with dental insurance, it is important to remember that you remain responsible for the stated fees. A payment will be expected on the day of service. The estimated benefit expected from your insurance plan will reduce this initial payment.

Our staff will file an insurance claim for you. After we receive payment from your insurance company we will send a statement if an additional payment is due.  You will be sent a refund if your account has a credit.

Scheduling

We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, please make sure that we are informed. Every attempt will be made to see you that day.

We try our best to stay on schedule to minimize your waiting. Due to the fact that Face and Jaw Surgery Center provides surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
