Patient Responsibility

  • It is the patient's responsibility to provide accurate and complete information about his/her present and past health care, including medications and other matters regarding his/her health.
  • The patient is responsible to fully participate in decisions that involve his/her own health care and to accept the consequences of these decisions if complications occur.
  • The patient is responsible for following the treatment instructions recommended by his/her doctor and as expressed by the office personnel as they carry out the plan of care.
  • The patient is responsible for fulfilling the financial obligations of their health care. He/she is responsible for furnishing information and cooperating with the expediting of payment by a third party payer.
