First Visit
As patients make their initial appointment, it will be discussed whether a consultation appointment or an appointment for oral surgery will be made. In either case, diagnosis and treatment options will be explained at the appointment. A complex medical history or complicated treatment plan will require treatment to be provided at a later date.
Patients may assist us by providing the following information at the time of their initial appointment:
- The surgical referral slip and any radiographs (x-rays), if not forwarded directly to our office by the patient’s dentist.
- A list of medications taken by the patient including prescription medications, doctor’s office samples, over-the-counter medications, vitamins, minerals, herbals, and nutritional supplements.
- Medical or dental insurance cards and/or forms. This will allow us to help process any claims for patients who have medical and/or dental insurance.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at their appointment.
